This was a super rare occurrence in the NHL, but there is an explanation.
Josh Anderson, first and foremost, threw a brutal check on Vegas defenseman Alex Pietrangelo and was subsequently tossed. A huge scrum took place, as you'd expect, and it led to a smorgasbord of penalties. Anderson took five-and-a-game for boarding, yet ironically it led to a Montreal powerplay.

So Montreal sent one guy to the box for five minutes, but Vegas sent two to the box for four minutes; just served simultaneously. Vegas would score on their powerplay shortly after and eventually win the game, so in that sense justice prevailed here. But for a second there, the hockey gods were asleep at the wheel, or at least it sure felt like it.
